ADA Accommodation Request

Use this form for disability-related accommodation requests pertaining to pre-employment interviews and use or participation in Missouri Department of Conservation (MDC) programs and facilities that are not addressed by the Special Use Permit. A Special Use Permit is required for people with mobility disabilities to use motorized vehicles (including ATVs, ORVs, golf carts, track chairs, and trucks) where public vehicles are not allowed on conservation areas.
